Family rates:2x the daily rates based on the oldest member of the family. Coverage dates must be the same for all family members and all family members must live at the same address while in Canada.
Maximum Period: including any extension(s),up to 85 age, 365days; 86 age and older, 180days.
Deductible:For age 85 or younger, $100 deductible applied, with 5% savings on the daily rate;$1000 deductible, with 25% savings on the daily rate;$3000 deductible, with 30% savings on the daily rate, only available for $100,000 and $150,000. For age 86 and older, $500 deductible applied.

Eligibility

Applicant must:

To be eligible for coverage under this plan, the applicant must:
1. Be a visitor to Canada, a person with a Canadian work visa or super visa, an immigrant to Canada or a Canadian resident, who is not eligible for a provincial or territorial government health insurance plan in Canada;
2. Not have been in Canada for more than 2 consecutive years prior to the policy effective date;
3. Be at least 15 days of age on the date of purchase;
4. Not be travelling against the medical advice of a physician and / or have been diagnosed with a terminal illness. A terminal illnessmeans that you have a medical condition that is cause for a physician to estimate that you have less than 6 months to live or for which palliative care has been received.
5. Not have a kidney disease requiring dialysis;
6. Not have Congestive Heart Failure, AIDS or require the use of home oxygen;
7. Not be experiencing new or undiagnosed symptoms and / or know of any reason to seek medical attention.
Note: Your spouse and / or child (ren) must also meet all the above criteria to be eligible for family coverage under this plan.

 

Features

Choice of policy maximum based on your needs :From $10,000 up to $150,000 CAD
Maximum $150,000 for age up to 69
Maximum $50,000 for age 70 to 85
Maximum $25,000 for age 86+

 

Deductible 

For age 85 or younger, $100 deductible applied, with 5% savings on the daily rate;$1000 deductible, with 25% savings on the daily rate;$3000 deductible, with 35% savings on the daily rate,only availble for $100,000 and $150,000. For age 86 and older, $500 deductible applied.

 

Maximum period of coverage: under 86, up to 365 days; 86 and older, up to 180 days 

 

Minimum premium: $25 CAD per policy

 

Family rate available
Twice the single rate based on the oldest member of family. children must be 21 year or under

 

Worldwide travel coverage
Provided the majority (51%) of your period of coverage is spent in Canada except the country of origin

 

Pre-existing condition
For age 69 and younger, automatically covers the Stable Chronic Condition If stable in the 120 days prior to the effective date of policy.

For age 70 to 85, Pay more for the Stable Chronic Condition If stable in the 120 days prior to the effective date of policy.

 

Waiting period
For policy purchased prior to your arrival in Canada: waived
For policy purchased after your arrival in Canada: 48 hours from the effective date of the policy for Emergency Sickness

 

Refund
Full refund is available
when the request for refund is received prior to the effective date of the Policy
Partial refund is available
If you return to your country of permanent residence
'No claim statement' and 'Proof of Return' (ex. airline ticket) are required
Requests for refunds must be made in writing within 90 days of your policy expiry date
An cancellation fee of $40 will apply to per policy
No refund will be issued
If claim has been or will be submitted
If the amount of premium to be reimbursed is less than $10 per policy

 

Benefits

Hospital Expenses: reasonable and customary cost for in and out patient treatment

Physician Fees

Diagnostic Services: reasonable and customary cost for X-rays and laboratory test

Ground Ambulance

Prescription drugs: up to $500 CAD, Limited to a 30-day supply per prescription

Medical Appliances
Casts, splints, trusses, braces, crutches, rental of wheelchair or other minor medical appliances

Paramedical Practitioner
Chiropodist, chiropractor, osteopath, physiotherapist or podiatrist up to $500 CAD per type of practitioner

Acupuncture: up to $500 CAD with a 365-day policy

Treatment of Dental Accidents: up to $3,000 CAD

Dental Emergency: up to $500 CAD

Air Fright Accident: up to the sum insured

Repatriation: when approved in advance by Global Excel

Return of deceased: up to $10,000 CAD

 

Claim

In the event of hospitalization, you must contact Global Excel within 48 hours of admission. Failure to make such notification will limit the total amount payable to 80% of all eligible expenses incurred;
